The health care delivery system in the United States does not work; it perpetuates unequal access to care, favors treatment over prevention, and contributes to persistent health disparities among the poor and racial-ethnic minorities. We develop effective, culturally responsive, and inclusive community health solutions, by developing and supporting partnerships between local community organizations, health care providers, social service organizations, and public health providers. Our community health science solutions combine community-oriented primary care (COPC), community-based participatory research (CBPR), and asset-based community
development principles. Our evidence-based programs embrace health as a right not a privilege, and emphasize relationship-based health care for all, population health, and the social determinants of health.

We build on the goals and objectives of our partners, and leverage local community assets, to increase health equity and reduce health disparities in urban and rural communities with high rates of preventable chronic illness. We have extensive experience creating equitable partnerships balancing the needs of health centers with the health priorities of community residents. During the past 30 years we have partnered with community leaders, faith-based organizations, non-profits, foundations, governmental organizations, health science centers, and universities for developing evidence-based approaches for improving health equity and eliminating health disparities. During the past 30 years we have partnered with community leaders, faith-based organizations, non-profits, foundations, governmental organizations, health science centers, and universities for developing evidence-based approaches for improving health equity and eliminating health disparities. Our health improvement research and training programs have been funded by the National Institutes of Health (NIH), Centers for Disease Control and Prevention (CDC), Exxon-Mobil Foundation, American Academy of Family Physician's Foundation (AAFP-F), United Way, and the American Cancer Society (ACS). The results of our efforts have produced tangible and dramatic improvements in community health outcomes. We disseminate and apply the results of our work throughout local communities and nationally through major peer-reviewed health journals and scientific conferences.
